Philhealth Claim Signature Form
- For this form, provide the employee’s name, their dependent, the date the patient was admitted and discharged, the name of the member’s representative (if applicable), the relationship of the representative to the member, your company signatory, and the reason for signing on behalf of the patient.
- Once you have done providing these details, simply click the Generate button, and check if all the needed details you have provided are accurate.
- You can click Print once you are done and use the form as needed.
Philhealth Form CF-1
- Provide the name of the employee needing this form, and provide information whether the patient is the member.
- For the Patient Information, you must provide the PhilHealth Identification Number of the Dependent, their complete name, date of birth, gender, relationship to the employee, and your company signatory.
- Once you have done providing these details, simply click the Generate button, and check if all the needed details you have provided are accurate.
- You can click Print once you are done and use the form as needed.
Philhealth Form ER2
- For this form, you must provide whether it is an initial or subsequent report. Please select the names of your employees in the report, how many employees per page will be shown, and your company signatory.

- Choose whether this form will be for your employee’s enrollment into the system or updating their information. Provide their name and the Formal Economy (Membership Category).
