PhilHealth Contributions Certification

  1. To generate this certificate, you will need the employee’s name, the company’s signatory, the year of the contribution payments, and the payroll type the employee is under. You also need to provide the payroll cycle or cycles that will be included in the report and the date of the certification’s publishing.
  2. Once you have done providing these details, you can click the Generate button and check if all the needed details you have provided are accurate.
  3. To get a copy of this report, click the Print button.
